This happened to my friend http://img158.imageshack.us/img158/1475/rofl2vj0.jpg When somebody on your friends list logs in, everything moves down one space to make room for your friend. So if right before you click, somebody logs in, you would click on somebody else's name instead of the person you want to send it to.
