You forgot to mention the weapon and/or shield.

The gear could do with a lot of improvement. Usually with PvM Werewolves, you either go Life Tap or mass life leech, you haven't got either. Remember that life leech works only at 25% efficiency in hell, so if you have 16% on all of your equipment, only 4% will work.

The weapon is crucial to your gear.

If you want to go Life Tap, use Last Wish Berserker Axe with Dracul's gloves, if you want to go mass leech, go for eBOTDz. If you already have reached your IAS breakpoints, throw in some Crushing Blow, for example, Steelrends instead of those crappy gloves that you are using. If you haven't reached IAS breakpoint, use Laying of Hands (Disciple set gloves), for nice damage to demons (most monsters in the game are demons) and extra IAS%.

Don't use Treks on a Werewolf, if PvM, definately go for Goreriders.

Instead of using Highlord, use Angelic duo for psycho AR, also you may consider using +4 skills Cerebrus instead of Jalal's.

2nd ring is up to you, if using Life Tap go for Raven or stat ring, if using leech go for a nice rare dual leech ring with sweet mods.

AR = Attack Rating, your chance to hit. Use Angelic Ring + Angelic Amulet for a mass AR bonus. Goreriders are cheap, as is Cerebrus, as is Ko. Also, calculate your IAS% breakpoint before you start.

Or you could do it yourself:
http://diablo2.ingame.de/tips/calcs/weaponspeed.php?lang=english

Don't bother putting any points into DEX, with Monarch Pheonix you will need tons, so abandon max block and use Angelic's for AR. If one Angelic ring is not enough, use both.

Use enigma definately because you need the frw bigtime and that allows you to get a lot more life because you don't have to waste on strength.

The best weapon by far for a wolf is a perf eth tomb reavere with (ed/ias) 40/15, 40/15 and zod.

also if u ever think u need to change ur gear, keep in mind the ias u have on ur weapon is the only ias that counts (i think fanactism counts too)
if ur using ebotd tmaul then i suggest u use enigma otherwise ur gonna need alot of str...
just use ebotdz+pheonix+fort, ull probably do as much dmg as with ebotdtm+enigma
(and zerker is faster than TM, right?)

u must have a shield because shields have chance to block which may save many of ur life points

The only weapon that can reach max ias for a wolf is a eth reaver.

Phoenix is useless for wolfs, it's mainuly used for zealers, amas and fire sorcs (rarely).

Use enigma ffs, you need the extra life and frw.
