Monsters have been changed greatly, with new skills and abilities. When was the last time a frog killed you? (The frog won't, the prince will!) Items have been overhauled for improved balance. There are no exceptional or elite items, but the base items get better when dropped by higher level monsters. That cap Baal insists on dropping may well have 300+ defense on it now.
The mod plays very differently from regular LoD. Monsters are fast and furious, defense is very important [hint: you know that defense is zero while running, but walk speed is very close to run speed now!], damage is high, but healing potions and skills provide rapid healing over time. And don't forgot the various delicious crowd control skills. Personally, I highly recommend this mod to all players. The new skills are incredible, along with the gameplay and kinds of items you can make. For the pros, there are Uber monsters (and levels) in the game that you can attempt (which have special drops in Hell, known in Median as Destruction, mode). This isn't one of those typical "click one monster and the entire map drops uber leet items" kind of mods; it will provide you with a fresh and new perspective on how you play your hero as he adventures his (or her) way through the evil that lies ahead...
